What is a thrombotic stroke?Thrombotic strokes are strokes caused by a thrombus (blood clot) that develops in the arteries supplying blood to the brain. This type of stroke is usually seen in older persons, especially those with high-cholesterol levels and atherosclerosis (a build-up of fat and lipids inside the walls of blood vessels).

The blood clot that triggers a thrombotic stroke usually forms inside an artery that already has been narrowed by atherosclerosis, a condition in which fatty deposits (plaques) build up inside blood vessels. A much less common cause of thrombotic stroke is migraine headache. In especially severe cases, a migraine headache can cause a brain artery to go into spasm for a long time, which can allow a blood clot to form.
Thrombotic strokes may be preceded by one or more "mini-strokes," called transient ischemic attacks, or TIAs. TIAs may last from a few minutes to a few days and are often a warning sign that a stroke may occur. Although usually mild and transient, the symptoms caused by a TIA are similar to those caused by a stroke. Another type of stroke that occurs in the small blood vessels in the brain is called a lacunar infarct. The word lacunar comes from the Latin word meaning "hole" or "cavity." Lacunar infarctions are often found in people who have diabetes or hypertension (high blood pressure).
The most effective treatment for stroke must be given immediately — within three hours after symptoms begin. For this reason, it is important to seek emergency treatment for anyone with symptoms of a stroke. The medicine is called tissue plasminogen activator (t-PA). It is administered through an intravenous line (IV). This medicine can dissolve clots and restore blood flow in the brain. Patients who receive this medication have less long-term disability following a stroke. Later, another type of medication, such as heparin, is given to prevent existing blood clots from getting bigger and to prevent new clots from forming.
